Medications: All medications-prescription and non-prescription must be brought to the school by the parent/guardian. Students are NOT permitted to transport any medication on the school buses/vans. Before any medication is given to your child, the medication consent form must be completed by the doctor and parent and brought in the original bottle (no meds in baggies). The medication is to be brought to school by the parent/guardian.

Physicals: Kindergarten, 6th grade, and 11th grade students are required by the Department of Health to have a physical. This may be done by your private physician or by the school physician. Private physical forms are available from this website (click on School Health/Consent Forms on the right margin and print!). A sports physical/driver's permit physical may be counted as an 11th grade physical.

 

Dentals: Kindergarten, 3rd, and 7th grade students are required by the Department of Health to have a dental examination. This may be done by your private dentist or by the school dentist. Dental forms are available from this website under school health/consent forms. 

 

Immunization Updates:  any time your child/children receive a new immunization, please ask your doctor for a copy of their current shot record so that his/her health file can be updated here at school! Please contact your healthcare provider or 1-877 PA HEALTH for more information about immunizations.

 

For Attendance in All Grades, Children Need the Following:

- 4 doses of tetanus, diphtheria, and acellular pertussis (1 dose on or after the 4th birthday)

- 4 doses of polio (4th dose on or after 4th birthday and at least 6 months after previous dose given)

- 2 doses of measles, mumps, rubella

- 3 doses of hepatitis B

- 2 doses of varicella (chickenpox) or evidence of immunity

 

7th Grade students: All those students who have entered 7th grade---you must have

1 dose of Tdap-(tetanus, diphtheria, acellular pertussis). and 1 dose of MCV (Meningococcal conjugate vaccine). 

 

12th Grade students: must have a second dose of the MCV (Meningococcal conjugate vaccine)
